Sempre Viva Aesthetics is a fictitious business name of California Aesthetic Physicians, PC, registered in Los Angeles County, California.

This Notice describes how Sempre Viva Aesthetics (“Sempre Viva Aesthetics,” “we,” “our,” “us”) collects, uses, discloses, sells, shares, and retains Personal Information of California residents when you visit our websites, request marketing materials, book appointments, communicate with us on social media, or otherwise interact with us outside of HIPAA‑covered treatment contexts.

Automatically from your device via cookies and similar technologies;
Service Providers & Contractors (payment processors, booking platforms, email/SMS vendors, ad networks);
Publicly available sources such as public social‑media profiles.

Subject to verification and legal exceptions, you may:

Delete Personal Information we collected from you;
Correct inaccurate Personal Information;
Opt Out of the sale or sharing of Personal Information for cross‑context behavioral advertising;
Limit use of Sensitive Personal Information (not applicable here);
Port your Personal Information in a readily usable format;
Non‑Discrimination: We will not deny goods/services, charge different prices, or provide a different level of quality because you exercised a right.

We verify requests by matching at least two data points you provide with data we maintain and may ask for a signed declaration. Authorized agents must present signed permission or power of attorney. We will acknowledge within 10 business days and respond within 45 calendar days (or up to 90 days with notice of extension).

Opt‑Out of Cookie‑Based Sharing

Click “Do Not Sell or Share My Personal Information” in the footer of our website or adjust preferences in the cookie banner to disable advertising cookies.

9. Data Retention

We retain Personal Information only as long as reasonably necessary to fulfill the purposes described in this Notice or as required by law. Typical retention periods:

Marketing contact data: 24 months of inactivity;
Transaction records (invoices, receipts): 7 years for tax & audit;
Website log files: 12 months for security and troubleshooting.

When retention ends, we delete or de‑identify the data.

10. Shine the Light (Cal. Civ. Code §1798.83)

California residents may request, once per calendar year, a list of third parties to whom we disclosed Personal Information for their direct‑marketing purposes and the categories shared. Use any contact method in Section 8.

11. Children

Our websites and services are not directed to children under 16. We do not knowingly collect, sell, or share Personal Information of children under 16. If we learn we have done so, we will delete that information promptly.

12. Changes to This Notice

We may update this Notice from time to time. The “Effective Date” above will change, and the updated Notice will be posted on our website. Material changes will be announced via a banner or email where feasible.
